If you are planning to submit an Academic Intervention Team/CARE Report for student attendance, please make sure that you have reached out to the student. For a sample email for student attendance/absences, please see AIT Attendance Email from Faculty.

LSU Student Advocacy & Accountability houses the C.A.R.E. Team, an important asset to the LSU community. By completing this form, you will be sharing your concern with a team of LSU Staff who are trained in responding to student needs. Faculty, staff, students, and others are encouraged to report any concerns they may have about a student. This can include academic performance issues (ex. failing grades, poor attendance, time management, etc.) and student behavioral concerns (ex. medical, psychological, behavioral, family crisis, financial, victimization, substance abuse/recovery, etc.). In the event of an Emergency, please do not use this form. Call LSU Police by dialing 911 or 225-578-3231. If you have any questions, please contact Student Advocacy & Accountability.
